Application – Due October 13, 2008
for the 2009-2010 ELC Team 

The following steps must be submitted for your application to be complete:

 1.  Complete the Application.  The application is comprised of basic questions and an essay section.  You may download the application or fill out this information online.

 2.  Three Reference Forms.  You must have three recommendations fill out the Alpha Phi Reference Form.  One of the recommendations must come from a member of your chapter’s Advisory Board.  The recommendations may be submitted in the following ways:

1)  The evaluator may send in the form directly to the Alpha Phi Executive Office.*

2)  The form may be in a signed and sealed envelope in the application package.

3)  The evaluator may sign and fax the form directly to the Executive Office (Attn: Gwendolyn Hill, fax: 847.475.6820)

*Please provide the evaluator a stamped envelope addressed to the Alpha Phi Executive Office. 

 3.  Professional Resumé and Photo.  Please be sure your application includes your professional resume and recent photo.  These materials may be emailed to ghill@alphaphi.org or mailed in with your application package.
